How about a renovation of the old slave holding building? I think the starting area should be reworked. Since you are in the middle of nowhere, you have to conserve water and food and shit. It could have been a great first challenge, like the mining station in Kotor2 or the dungeon in BG2 that both were much more than just a tutorial.
Fuck, throw in 3 or 4 extra maps, and some old crazed hermit or something with a ministore with only crap items, maybe move that smelly cat lady from next to the swamps to this wilderness instead and make her teach you forage (forage does decrease the amount of food and water you drink, or did I get that wrong?) instead of medicine and repair instead of alchemy. And then some goblin-slaying (non-essential) miniquest.
In fact, I think the game was slimmed to much. In both E1 and E2, there are four maps between the first and the second waypoint. In E3, there is almost one in every map... in fact, one of the maps have 2 waypoints! (Well, E2 did too, but at least they were separated by a mountain). So, extra nature fucken everywhere.
Now, I do realize that takes a lot of work, but this thread IS a wishlist after all, so...

Most of the time my underdeveloped foraging skill gifted me with bitterseeds and cabbages, from time to time I was even able to find some water (should not be too hard when camping riverside, even without the foraging skill, though). But more than a decrease of the thirst bar never seemed to happen, or is it possible to refill your water skin at a higher level?

Yes - the complaint with Forage in Book II was that it was "game-breaking", because "slowing the rate of decline" of the Hunger and Thirst bars proved to be too powerful. FTR, I vastly prefer the Book III implementation, with the caveat that I think Level 1 in Forage should only yield up Bittercup Seeds - Cabbages are actually a powerful food (alleviating both Hunger and Thirst), and shouldn't be achievable until higher levels in Forage.
Personally, I think Forage should go something like this (e.g. in v1.02):
Level 1 - Bittercup Seeds only (and infrequently found)
Level 2 - Bittercup Seeds & Plumpcots (only slightly less infrequently found)
Level 3 - Bittercup Seeds, Plumpcots & Yambi Berries (somewhat infrequently found)
Level 4 - Bittercup Seeds, Plumpcots, Yambi Berries & Cabbages (somewhat less infrequently found)
Etc.

Lilith romance mod;
Create some new fetch / kill quest or whatever that Lilith wants you to perform, and if you afterwards refuse the reward, you get the dialogue optoin to butter her up (verbally). Then, if you fork over the engagement ring (found quite lab in the game and have no useful purpose other than selling) and flatter her again, she will pretend to be offended but keep the ring. In the end cut scenes, when you you will be seen together with her.
If you play a girl same thing only you date some hot dude like... err... Kirk Strongarm?

Eschalon stealth tweaks:
* Hide and move silently is made into one skill, Stealth.
* Both Dexterity and Perception contributes to Stealth.
* Staying silent is easier (requires less of stealth skill) than staying hidden...
*...but hiding can be enhanced with dark clothing (new material, less protection / durability but increase chance of hiding).
*...and chamelon spell, 1 stealth skill point / power level
*...equipping knives, throwing weapons or stay unarmed does not decrease stealth, but all other weapons do.
*...attacking an unaware enemy increase damage 1,5x UNLESS you perform a feat.
*...piercing weapon feat have a speed% chance of outright killing all enemies except boss type monsters and undead if the enemy is unaware
*...throwing weapons feat have 1/2 speed% chance of -II-
*...bows feat have 1/2 concentration% chance of -II-
*Special "dark enhancement" alchemy recipe (like ectoplasm + poisonus mushroom) to piercing weapons that gives +x to stealth and +x to chance of assasinate with piercing weapons feat
*invisibility spell is removed, you can only be invisible through potions. Since they require ectoplasm, that quickly becomes an expensive ordeal.

Same - that was too good. I do think that, were I a girl, I would choose Malkur over Kirk Strongarm. Maybe even Jawbreaker or Crazy Bernie over Kirk Strongarm...

I would go as far to say that I would choose the Akadai Kingspawn over Kirk Strongarm.
